We are currently searching for more Patient Assessment Coordinators to join our Rehab team! The Patient Assessment Coordinator coordinates and collaborates with the interdisciplinary team for the completion, gathering and input of data, and completes the transmission of data to the appropriate government or regulatory agency. The Patient Assessment Coordinator designs data collection procedures and methodology through the analysis and reporting stages, as required, ensuring compliance with internal policy and external regulatory requirements. The Patient Assessment Coordinator provides designated compliance coordination, and may provide direct patient care, as required. Schedule: 8:30am - 4pm - about four days per week (days determined based on business needs) Essential Job Functions:* Develop and/or modify data collection and information management systems to ensure Patient Assessment compliance with all federal requirements and internal organizations standards of quality, accuracy, and timeliness.* Participate in required training and maintain proficiency with all applicable Prospective Payment System (PPS) Patient Assessment regulations and serve as the designated organizational resource person in PPS and Patient Assessment related matters; conduct random medical records audits as required to ensure accurate, consistent documentation between Patient Assessment and medical records.* Complete the required transmission of data independently; track atypical cases as required; and communicate essential patient basement data to the Rehabilitation Facility clinical and operational leadership, including the utilization review, finance, and billing office areas.* Review and assess Quarterly Compliance reports received from any governmental or regulatory agency for audit prevention and quality improvement measures.* Coordinate the gathering, aggregation, analysis, trending, and reporting of outcomes-related information.* Coordinate organizational compliance to the information and outcomes management section of the Commission on Accreditation of Rehabilitation Facilities standards and other applicable regulatory requirements.* Provide and participate in required training as it relates to outcomes management systems, application, implementation, and reporting.
