GENERAL SUMMARY AND PURPOSE: Coordinates services to support Community Health & Well-Being (CHWB) initiatives. Supports and promotes the Mission of Saint Alphonsus through strategic and programmatic outreach within Saint Alphonsus Health System service area. Responsible for orientation, education, and training of pertinent staff and/or volunteers to the specifics of the CHWB initiatives and represents Saint Alphonsus in related community coalitions. Promotes Saint Alphonsus Health System, as appropriate, on the local, state and national level. Serves as a liaison between the community, community agencies, and professionals dealing with the health and well-being of populations served. SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE: Bachelor's degree in Health Care Administration, Public Health, Community Health or a health-related field with one (1) to three (3) years of experience community health, public health, social services; or an equivalent combination of education and experience. Minimum of 2 years of experience required, or certification or specialized education in the related field (E.g. community health outreach, community benefit accounting, seniors or other vulnerable persons outreach). Program development/coordination and/or staff/volunteer coordination or oversight experience preferred. Master's degree in related field, preferred. ESSENTIAL FUNCTIONS: Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions. Collaborates with other health and community agency professionals, patients, clients, families, administrators, and caregivers to meet the health and developmental needs of populations served. Supervises any assigned staff and/or volunteers. Document, compile and/or analyze care and/or community benefit data to demonstrate work and outcomes of program and to plan for program community benefit budgeting. Assists in planning and implementing community benefit and CHWB programs in response to the CHNA and Trinity Health strategies. Manage timely and accurate coordination and documentation for internal policy and external regulatory requirements (i.e. Community Benefit in accordance with Catholic Health Association and IRS regulations) Serves as a resource and provides related CHWB training for stakeholders, colleagues and providers for CHWB. Engages the community and participates in coalitions. Supports the implementation of triennial Community Health Needs Assessment (CHNA) including but not limited to helping convene stakeholders, develop survey materials, collect data, and write and share reports Demonstrates good communication (interpersonal, verbal and written) skills Demonstrates basic computer skills. Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.
