GENERAL SUMMARY AND PURPOSE:

A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

High school diploma or general education degree (GED); or one to three years related experience and/or training; or equivalent combination of education and experience. Basic Life Support for Healthcare providers certification from either AHA, ARC or the Military Training Network is required at hire.

If based in Oregon, must be able to obtain Community Health Worker (CHW) in Oregon within 6 months of hire. Bi-lingual skills to work with patient populations in their native language preferred, but not required. Spanish speaking communication skills strongly preferred. Must have active driving privileges. If, during the course of employment, driving privileges change or colleague receives a driving citation, the colleague is required to notify their supervisor and/or HR.

ESSENTIAL FUNCTIONS:

Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health. Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence.

Ability to effectively interview clients and accurately record medical information. Ability to effectively present information in one-on-one and small group situations to clients, and other employees of the organization. Ability to effectively relate pertinent information with medical and social service professionals.

Must possess a comprehensive knowledge of the local community Facilitates or conducts enrollment in available federal, state and local programs to increase access to health care and support services. Examples include: health coverage via the marketplace, Medicaid, Medicare, social security benefits, food, and housing services.

Provides in-service education to service providers relative to healthcare customs/beliefs of the target population and works cooperatively with other community agencies to promote culturally appropriate services.

Maintains frequent contact with community agencies serving vulnerable populations and participates in community meetings. Other duties as needed and assigned by the manager.

Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health's Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior. Able to recognize and respect the limits of their skills, abilities and boundaries of the role.

Bi-lingual English and Spanish strongly preferred.

Clarifies with supervisor as situations arise. Work requires ability to communicate orally and to hear or communicate with target population.

Community Care Coordination

Provide effective care coordination for individuals

Build caring relationships with participants to promote positive health and social service outcomes

Adhere to an ethical code of principles to strengthen the professional role of the CHW

Culturally Competent Interpersonal & Communication Skills

Individual & Community Advocacy - Serves as a community liaison between target populations and agencies and providers

Teaching Strategies for Change

Assess stage of change and provide education appropriate for readiness

Utilize a variety of teaching strategies, including verbal, handouts, and/or video based on participant learning style

Individual & Community Outreach - Recruit and engage program participants

Community Resources

Identify and refer people from targeted populations with basic needs

Utilize community resources and their referral processes

Identify and utilize entitlement participant program services

Collecting & Reporting Confidential Information - Understands and is capable of using multiple technologies including but not limited to desktop/laptop computers, mobile devices and tablets in order to accurately and in real-time capture relevant social determinant information for the medical record

Organizational & Personal Skills

Identify the emotional dynamics involved in CHW work and utilize a personal and professional support system to cope with these dynamics

Practice efficient time management and document time allocation accurately

Perform basic clerical, computing and office skill
